WHAT IS CARE SIGNALS

Always-on AI for safeguarding and documentation confidence.

Care Signals is ShiftCare's AI documentation confidence engine for NDIS providers. It reads every progress note and shift form response in real time, against the shift it covers and the client's profile, then routes the ones that need attention to a single triage queue. Safe for audit. Safe to bill. What needs action. Always on. Always reading. Always working in the background.

AI scanning every NDIS progress note in real time to flag safeguarding risks for triage
REAL-TIME SAFEGUARDING

A safety net under every progress note.

Incidents rarely arrive with the word "incident" attached. Support workers describe what they saw in plain language, so the most serious concerns can sit unread for days. Care Signals reads every note in real time against the shift it covers and the client's profile, tags concerns by severity (High, Moderate, or Low) with a confidence score, and routes the ones that matter into a single queue.

Automated note review

Every note, automatically reviewed

AI reads progress notes and shift form responses as your team writes them. No extra steps for support workers, no behaviour change required.

Reads in context

Reads in context, not just keywords

Every note is read against the shift it covers, the client's medications, care plan tasks, support needs, and known triggers. Detection adapts to who the client is and what was supposed to happen.

Three severity levels instantly tagged

AUDIT AND BILLING CONFIDENCE

Safe for audit. Safe to bill.

Documentation rolls up to two practical worries for NDIS providers: defending it under audit, and billing against it without rejection or clawback. Care Signals runs the same review you'd run before either question gets asked of you, checking every note for empty sections, vague entries, and boilerplate phrasing alongside the safeguarding flags. Built to the NDIS Quality and Safeguards Commission's incident management standards.

Empty sections flagged

Empty sections flagged

Required template headings left blank, filled with placeholders like "N/A", or with filler like "ok" get surfaced before they become an audit finding.

Insufficient detail flagged

Insufficient detail flagged

Notes too brief to convey what happened get caught early. Detection calibrates against shift length, so a short note is fine for a 15-minute prompt, not for an 8-hour shift.

Boilerplate flagged

Boilerplate flagged

Generic phrasing like "all tasks completed" or "no concerns" gets called out, so it doesn't quietly weaken your billing position.

Full audit trail

Full audit trail

Every flag and every triage decision is logged with timestamp, user, and action. Nothing is permanently deleted.

Care Signals triage queue with one-click incident creation for NDIS admins
quote

Care Signals has been a huge win for our team. It was enabled on our account on Friday and has already picked up key client presentation notes that admin staff had missed, helping us confirm incidents and WATCHOUTS faster. It’s also a strong selling point for our aged care services. Huge thanks to Paul and the team. We’re loving the work you’re doing.

Any Questions?

What is Care Signals?

Care Signals is ShiftCare's AI safeguarding and incident detection layer. It reads every progress note and shift form response as it's submitted, flags incidents and safeguarding risks in real time, and routes them to one triage queue with severity tags and confidence scores. Manual review depends on admins reading thousands of free-text notes weekly. Care Signals reviews every note automatically, so nothing slips through.

What context does the AI use when reading a note?

+

For every note, Care Signals reads against the shift it covers (start, end, duration, overnight status) and the client's profile, including active medications, care plan tasks scheduled for the day, support needs, active goals, known triggers, and preferred coping strategies. It also reads against your account's own mandatory note template. This is what lets Care Signals answer the question an auditor actually asks: did the worker document what was supposed to happen for this client on this shift?

How does Care Signals help with audit and billing readiness?

+

Care Signals checks every note for three documentation completeness rules alongside the safeguarding flags: empty mandatory sections (template headings left blank or filled with placeholders), insufficient detail (notes too brief for the shift length), and generic boilerplate (phrases like "all tasks completed" without specifics). Notes that don't hold up to an audit, or don't clearly back up the work delivered for billing, surface in the same triage queue as risk flags, so admins can tighten them up before either question gets asked.

Does Care Signals meet NDIS Commission incident management requirements?

+

Care Signals is built to support NDIS Quality and Safeguards Commission obligations for incident identification, response, and reporting. It flags reportable incident types automatically, logs every triage decision in a full audit trail, and integrates directly with ShiftCare's Incident Management product so confirmed incidents can be progressed to Commission notification.

What types of incidents does Care Signals detect?

+

Care Signals detects three severity levels. High severity covers emergencies, falls, injuries, aggression, medication errors, self-harm, and safeguarding concerns. Moderate severity covers behavioural changes, transport incidents, client distress, and acute health symptoms. Low severity covers routine operational issues. Documentation quality flags are also generated for empty fields, vague entries, and boilerplate language.

Can I customise what Care Signals flags?

+

Yes. From AI Settings you can enable or disable individual categories, adjust confidence thresholds for each one, and add custom keywords or category descriptions to match your service model. Detection rules are tunable per account, so the system reflects how your organisation defines a concern.

Do support workers need to do anything differently?

+

No. Support workers write progress notes and shift form responses the same way they always have. Care Signals reads notes in the background, so there's no new field to fill in, no checkbox, and no workflow to learn. The change happens on the admin side, where flagged notes appear in the triage queue automatically.

Where does my participant data go?

+

Care Signals reads notes inside your ShiftCare account for detection only. Participant information isn't stored externally or shared outside your account. Full audit trails are kept inside your account for compliance purposes, and nothing is permanently deleted from the flag history.
